Our UChicago Medicine Post-Discharge Clinic is here to help patients when they need follow-up care after a recent hospital stay.

What is our Post-Discharge Clinic?

We provide transitional care tailored to your needs and put your continued health and safety first. We aim to:

  • Reduce the length of stay of your hospitalization.
  • Improve the early identification of adverse events, helping prevent readmissions.
  • Enhance patient education and experience.
  • Produce a smooth transition of care back to your primary care physicians.

How does our Post-Discharge Clinic work?

Your journey with our care team begins with a smooth transition from discharge from the hospital to the post-discharge clinic appointment and from the clinic to your primary care doctor. Our providers and staff will work with you to set up a plan for success and schedule future appointments.

Around two days after discharge, a nurse will contact you via phone call to follow-up. Within the first 14 days, you will have your appointment at our clinic. We coordinate your care until you see your primary care doctor.

We will partner with your primary care doctor to keep them informed on your health and ensure a smooth transition for the care you receive.

What can you expect during your visit to our clinic?

While at your appointment, our doctor can assist you with a variety of ways, including:

  • Assisting with pharmacological interventions.
  • Reviewing and refilling prescribed medications.
  • Ordering or following up on ancillary tests and discussing the results with loved ones.
  • Arranging secondary follow-up appointments for patients who need to be seen again.
  • Coordinating referrals with other physicians when a specialized level of care is needed.
  • Offering patient self-care education and resources.

In-person visits

We also offer in-person visits at our UChicago Medicine Duchossois Center for Advanced Medicine - Hyde Park location. We are located on the 3rd floor at 5758 South Maryland Avenue, Chicago IL 60637. Parking is available in parking lot A. You can check in at desk 3C.

Virtual video visits

We use the latest technology to offer video visits. This safe and convenient appointment method makes it easy to connect to our care team. We use MyChart for video visits. If preferred, we can use Doximity to text you a link to connect via video.

If you are unable to log into MyChart at the time of your visit, please expect a text from your provider to use Doximity. There is no application download needed for Doximity.
